В Санкт-Петербурге со 2 по 4 октября в Екатерининском дворне прошел II Всероссийский Конгресс по слуховой имплантации с международным участием под названием « Слуховая имплантация в России », на который приехали более 350 отечественных и иностранных специалистов, а также представителей общественных организаций России.

В Санкт-Петербурге со 2 по 4 октября в Екатерининском дворце состоялся второй по счету Всероссийский Конгресс «Слуховая имплантация в России». В нем приняли участие оториноларингологи, специалисты по реабилитации речи, сурдологи, аудиологи и сурдопедагоги практически из всех регионов России. Организатором мероприятия выступил Санкт-Петербургский научно-исследовательский институт уха, горла, носа и речи (СПб НИИ ЛОР) при поддержке Министерства здравоохранения и Российской академии медицинских наук. Со своими докладами на Конгрессе выступили не только российские специалисты, но и врачи из ведущих центров по кохлеарной имплантации Европы: Австрии, Германии, Польши, Греции. Словакии, а также США, Канады, Бразилии, Среднего и Дальнего Востока.
Познакомиться с новейшими тенденциями в КИ
Кохлеарная имплантация, технология которой постоянно развивается, призвана помочь тем, кому не помогают самые современные слуховые аппараты. В программу Конгресса вошли научные доклады, семинары и дискуссии, посвященные новейшим мировым тенденциям в этой области. Здесь обсуждались такие вопросы, как эволюция хирургических подходов к проведению кохлеарной имплантации, бинауральная имплантация, имплант костной проводимости, применение имплантов среднего уха в лечении сенсо-невральной и кондуктивной тугоухости и мн. др.
«Во-первых, это была возможность подвести итоги проделанной работы за последние два го-да,-подчеркнул Юрий Янов.- Если по количеству выполненных кохлеарных имплантаций мы почти сравнялись с европейскими и северо-американскими показателями, то стройной системы реабилитации данной группы больных у нас еще нет. Поэтому основной тематикой конгресса стала проблема реабилитации больных после кохлеарной имплантации. Это очень сложная задача: необходимо подготовить кадры сурдопедагогов и логопедов, обучить их специальным программам, организовать постоянное повышение квалификации ».
Кохлеарная имплантация в России
«Технология кохлеарной имплантации постоянно развивается.»
Согласно статистическим данным на 2010 год, в России проживает около 13 миллионов глухих людей, среди них насчитывается 1 миллион детей. Считается, что на каждые 1000 родов рождается один глухой ребенок, в младенческом возрасте 2-3 ребенка теряют слух вследствие инфекций или осложнений от лечения. «За год в России проводятся около 1400 операций, из них 95% - в центрах Москвы и Санкт-Петербурга. Наш СПб НИИ ЛОР по итогам 2011 года занял первое место по количеству выполненных кохлеарных имплантаций.-сказал Юрий Янов.- Можно с ответственностью сказать, что СПб НИИ ЛОР и Московский Научно-Клинический центр оториноларингологии выполняют сегодня подавляющее количество КИ по стране. Бинауральная имплантация в стране развита пока еще недостаточно, но надеемся, что 2013 год станет переломным».
Развитие технологии КИ в различных регионах России
Операции кохлеарной имплантации производятся, в основном, в Москве и Санкт-Петербурге. Московский центр оториноларингологии открыл свои филиалы также в Томске, Уфе. Хабаровске и Астрахани. «Специалисты СПб НИИ ЛОР активно сотрудничают с Челябинском и Красноярском.-уточнил профессор.- Нам удалось наладить дистанционную настройку кохлеарных имплантов в Красноярске, благодаря чему больные могут не совершать далекие и дорогие путешествия в наш город, что очень важно». Прежде всего, основная проблема реализации программы кохлеарной имплантации в России - это отсутствие подготовленных кадров в регионах. «Если хирургическое вживление имплантов в крайнем случае, можно обеспечить силами ведущих центров Москвы и Санкт-Петербурга, тс реабилитация, обучение речи, социальная адаптация должна осуществляться исключительно т местах»,- подчеркнул Юрий Янов. И в этом на правлении необходимо еще очень много сделать
Решить проблему финансирования на федеральном уровне
На Конгрессе поднимался вопрос, волнующий всех пациентов - гарантированные сроки бесплатной замены речевых процессоров и их ремонт. В настоящее время проимплантированные инвалиды сталкиваются с тем, что после окончания гарантийного периода эксплуатации элементов внешней части системы кохлеарной имплантации дальнейший ремонт и замену системы они должны оплачивать самостоятельно. Однако, ни в oдном из нормативно-правовых актов РФ не указе но, что после проведения операции по кохлеарной имплантации в рамках высокотехнологичной медицинской помощи последующий ремонт и замена на должны проводиться за счет инвалидов.
На сегодняшней день в различных регионе России (Курская. Воронежская, Рязанская, Московская области), как подчеркнули в своем докладе Сергей и Наталия Сазоновы (ГБОУ ВП «Юго-Западный государственный университет» складывается ситуация, когда чиновники органов соцобеспечения отказываются обеспечитьфинансирование содержания, ремонта и замены внешних частей системы КИ за счет средств федерального бюджета и пытаются всеми силами переложить это на плечи родителей детей-инвалидов, самих инвалидов, различные благотворительные организации или на региональный бюджет. Есть ряд регионов, в которых это финансирование производится за счет средств федерального бюджета. Это, например, Ханты-Мансийский автономный округ, Республика Саха (Якутия), Новосибирская область. Выступающие подчеркнули, что «для решения возникшей проблемы на федеральном уровне необходимы корректировки или дополнения к существующим НПА, которые однозначно определяли бы, что кохлеарный имплант - это эндопротез, состоящий из внешней и внутренней частей, и что финансирование ремонта и замены внешней и внутренней частей должно производиться за счет федерального бюджета вне зависимости от того, проведена операция по кохлеарной имплантации за счет квот ВМП или за счет средств органов соцобеспечения».
Правильная реабилитация - залог успеха
Безусловно, одной кохлеарной операции недостаточно, чтобы помочь ребенку слышать. Чтобы научить ребенка «пользоваться» собственным слухом, необходима реабилитация, иначе все усилия врачей и государства могут оказаться напрасными.
«Реабилитация после кохлеарной имплантации имеет свои особенности.»
«К сожалению, можно сказать, что контроль за оказанием медицинской помощи пациентам после КИ выполняется пока еще плохо.- подчеркнул Юрий Янов. - Поэтому задача нашего Конгресса- сдвинуть ситуацию с мертвой точки и наладить жесткий медицинский контроль не только за расходованием средств, но и за результатами реабилитации. Каждый плохо реабилитированный больной - это выброшенные на ветер деньги нашей и без того не богатой медицины». Реабилитация пациентов после кохлеарной имплантации имеет свои особенности, это обучение навыкам речи практически с нуля. Поэтому этот процесс является крайне трудоемким и требует годы кропотливой работы. Число имплантируемых детей растет быстрее, чем темпы организации реабилитации в регионах. А задача врачей - подготовить из глухих детей-инвалидов полностью социально адаптированных людей.
Необходимо подготовить кадры сурдопедагогов и логопедов, обучить их специальным программам, организовать постоянное повышение квалификации.
Комплексное сопровождение детей после КИ
Третий день конгресса был посвящен реабилитации детей после КИ и их интеграции. За круглым столом экспертов обсуждались методы и приемы коррекционной работы при обучении детей после имплантации, тип образовательного учреждения, который должны посещать дошкольники и школьники с КИ, а также необходимые педагогам и родителям материалы для работы с детьми с КИ.
«К сожалению, в данном вопросе реабилитации и социализации детей после операции кохлеарной имплантации отсутствует соответствующая нормативно-законодательная база,- подчеркнул профессор. - Наши законотворцы должны срочно нивелировать разрыв между уникальным научным прорывом в области восстановления слуха у полностью глухих людей и устаревшими на сегодняшний день нормативными актами». Вопрос интегрированного обучения глухих детей после кохлеарной имплантации является первоочередным. Ребенок должен перестать считать себя инвалидом. В условии специальных школ и интернатов для глухих это. К сожалению, невозможно. Необходимо успешно реабилитировать ребенка, и как можно быстрее направить его в обычную школу, то есть помещать реабилитированных детей в обычные группы. Специализированное учреждение для глухих замедляет процесс реабилитации. На конгрессе поднимался вопрос комплексного сопровождения ребенка с нарушением слуха после кохлеарной имплантации. Это комплексная программа, включающая в себя работу сурдопедагогов, логопедов, психологов, а также инженеров, аудиологов. сурдологов. вовремя вносящих коррективы в работу импланта.
Участие государства
В Доме молодежи «Царскосельский» было организовано совещание в формате Круглый стол. в котором приняли участие уполномоченные по правам человека в регионах РФ, представители Совета Федерации РФ, Государственной Думы, Фонда социального страхования. Министерства здравоохранения. Министерства труда и социальной защиты и др. официальные лица. На нем обсуждались вопросы качества и контроля оказания медицинской помощи, правовые, социальные и экономические аспекты обеспечения техническими средствами реабилитации пациентов после операции, в частности, тема комплексного подхода к организации слухоречевой реабилитации пациентов после КИ, обеспечивающего доступность, единообразие стандартов технологии и критериев качества реабилитации на всей территории России.
Председатель комитета Совета Федерации по социальной политике Валерий Рязанский подчеркнул, что необходимо активное участие государства в реабилитации детей с установленными имплантами. Сегодня в России государство берет на себя расходы на кохлеарную имплантацию детей, но после операции за восстановительное лечение родителям приходится платить самим. Отдельно был выделен круг вопросов, касающихся защиты прав людей с ограниченными возможностями по слуху. «Это очень щекотливый круг социальных гарантий, - уточнил Юрий Янов. - Человек, перенесший кохлеарную имплантацию и удачно реабилитированный, теоретически может лишиться социальных льгот, на которые рассчитывает и к которым привык». На конгрессе обсуждались правовые социальные и экономические аспекты обеспечения техническими средствами реабилитации. «Участие в Конгрессе представителей Совета Федерации, различных министерств и ведомств является гарантией того, что все поднятые вопросы будут решены», - добавил Юрий Янов.
Значение проведения Конгресса для развития КИ в России
Значение данного Конгресса трудно переоценить. Это. во-первых, обмен опытом, в том числе, и международным, ведь в Конгрессе приняли участие специалисты из ведущих центров по кохлеарной имплантации из разных стран мира. Во-вторых, благодаря широкому участию оториноларингологов и специалистов по реабилитации речи, сурдологов. аудиологов, сурдопедогогов, приехавших со всех концов России, это внедрение опыта в регионы, а в-третьих, это и нормативное, законодательное закрепление полученных результатов при помощи представителей администрации, которые принимали участие в Конгрессе. «При существующей социальной поддержке различных комиссий Парламента. Министерства Здравоохранения, Региональных органов власти будущее кохлеарной имплантации видится в самом привлекательном свете, - подвел итоги Юрий Янов,- За короткое время мы смогли достичь в этой области уровня ведущих западных держав, теперь наша задача - не снижать достигнутых показателей и двигаться дальше». Второй Всероссийский Конгресс стал еще одним шагом к объединению профессионалов в раз витии высокотехнологичной помощи людям с ограниченными возможностями по слуху, затронув важные вопросы реабилитации пациентов с имплантами.

Learning about the latest trends in CI
Cochlear implantation is a technology undergoing constant development, and is used to treat people who cannot benefit from the most modern hearing aids. The program of the Congress included scientific papers, seminars and debates devoted to the latest global trends in this field. These involved discussions of issues such as the development of surgical approaches to cochlear implantation, binaural implants, bone conduction implants, the use of middle ear implants to treat sensorineural and conductive hearing loss, and many other topics.
"First of all, this was an opportunity to report on the results of work carried out over the last two years", Yuri Yanov emphasizes. "Whilst in terms of the quantity of cochlear implants carried out we have almost equaled European and North American numbers, we do not yet have a well-developed system of rehabilitation for this patient group.
For this reason, the main theme of the Congress was the problem of patient rehabilitation after a cochlear implant procedure. This is a very complicated task: it is necessary to train teachers of the deaf and speech therapists, educate them in specialist programs and organize continuing professional development."
Cochlear implants in Russia
According to statistical data from 2010, there are approximately 13 million deaf people living in Russia, about 1 million of whom are children. It is believed that out of every 1000 births, one deaf baby is born, and that during infancy two to three children lose their hearing as a consequence of an infection or complications following treatment. "Each year approximately 1400 operations are performed in Russia, of which 95% take place at centers in Moscow and Saint Petersburg. According to the figures for 2011, our SP RI ETNS carried out the highest number of cochlear implant procedures". Yuri Yanov says. "I can say with certainty that between them, SP RI ETNS and Moscow Scientific and Clinical ENT Centre carry out the lion's share of CI procedures in this country." The use of binaural implants is still insufficiently developed in Russia, but we hope that 2013 will be a turning point for this.
Development of CI technologies in various regions of Russia
Cochlear implant operations take place mostly in Moscow and Saint Petersburg. The Moscow ENT Centre has also opened branches in Tomsk, Ufa, Khabarovsk and Astrakhan. "Specialists from SP RI ETNS are working actively with Chelyabinsk and Krasnoyarsk", the professor comments. "We have successfully implemented the remote adjustment of cochlear implants in Krasnoyarsk, thanks to which patients no longer have to embark on the long and time-consuming journey to our city, so this is very important."
Above all else, the main problem in implementing the cochlear implant program in Russia is the lack of trained staff in the regions. "While the surgical implantation of implants can, if really necessary, be carried out at the leading centers in Moscow and Saint Petersburg, the process of rehabilitation, speech training and social adaptation absolutely must take place locally", says Yuri Yanov. And in t1 area, a great deal still needs to be done.
Solving the problem of funding at the federal level
At the Congress, an issue of concern to all patients came up - warranty periods for free exchanges and repairs of speech processors. At the present time, people with implants have to face fact that after the expiry of the warranty period for the elements of the outer part of the cochlear implant system, any further repair and exchange of the system must be paid by them independently. However, nowhere in Russia's regulatory legal framework does it slate that after carrying out a cochlear implant operation as part of high-tech medical treatment should subsequent repairs and exchanges be carried out at the patient's expense.
At present, in several regions of Russia (Kursk, Voronezh, Ryazan and Moscow region), as pointed out in the paper given by Sergey Sazonov and Natalya Sazonova (GEOU VPO South-West State University), the situation is occurring that social security officials refuse to agree to provide financial support for maintenance, repairs and exchanges of external parts of CI systems from the federal budget and try with all their might to shift this responsibility onto the shoulders of the parents of disabled children, the disabled persons themselves, various charitable organizations, or the regional budget.
There are a number of regions in which this funding comes from the federal budget. These include Khanty-Mansi Autonomous Okrug, Sokha Republic (Yakutia) and Novosibirsk Oblast.
The speakers emphasized that "resolving this problem at the federal level will require adjustments or additions to be made to the current regulatory legal framework, which should unambiguously define a cochlear implant as an endoprosthesis consisting of external and internal parts, and that funding of repairs and exchanges ot the external and internal parts must be charged to the federal budget, regardless of whether the cochlear implant operation was paid for from quotas for high-tech medical treatments or social security funds".
The right rehabilitation process is the guarantee of success
Undoubtedly, on its own a cochlear implant procedure is not sufficient to help a child to hear. Teaching children to "make use" of their own hearing requires a process of rehabilitation, otherwise ail of the doctors efforts and the state's expenses may turn out to be in vain. "Unfortunately, it must be said that monitoring of Itie medical care given to patients after the CI procedure is still not good", Yuri Yanov states. "For this reason, our Congress's task is to get things moving and implement strict medical monitoring, not only of the use of funds, but also of the results of rehabilitation. Each time a patient is rehabilitated poorly, it means our money is lost to the wind, and affordable medicine along with it." The rehabilitation of patients after a cochlear implant operation has its own distinctive features - it consists of teaching the skifl of forming speech almost from zero. For this reason, this process is extremely labour-intensive and requires years of painstaking work. The number of children with implants is growing more rapidly than the rate at which rehabilitation services are being set up in the regions. And yet the task placed on doctors is to help disabled children to grow into fully socially-adapted people. It is essential for us to train workforces of teachers of the deaf and speech therapists, educate them through specialist programmes and organize continuing professional development.
Children require complex support after CI
The third day of the Congress was devoted to the rehabilitation and social integration of children after receiving CI. A round table of experts discussed methods and techniques of corrective work as part of training children following implantation, the type of educational establishment that should be attended by pre-school and school-age children with CI, and also the kinds of materials needed by teachers and parents for working with children with CI. "Unfortunately, in this issue of the rehabilitation and social integration of children after receiving Ci, the corresponding regulatory and legal framework is missing", stresses the professor. "Our lawmakers urgently need to bridge the gap between the unique scientific breakthrough in the recovery ot hearing in completely deaf people and the outdated regulatory framework.'' The issue of integrated education for deaf children after receiving cochlear implants is a priority. The child must stop seeing him or herself as disabled. However, when studying at special schools and boarding schools for the deaf, this is unfortunately not possible. It is essential to rehabilitate these children successfully and then send them as quickly as possible to mainstream schools, in other words integrate rehabilitated children into regular groups. Specialized establishments for the deaf slow down the process of rehabilitation. At the Congress, the issue arose of the complex support needed by о child with a hearing impairment after receiving a cochlear implant. This consists of a complex program involving the work of teachers of the deaf, speech therapists, psychologists, engineers and audiologists, each at the appropriate time making their contribution to the implant working correctly.
State participation
At Tsarskoselsky Vouth Building, a meeting was held in the form of a round table, attended by human rights representatives from Russian regions and representatives of the Federation Council and State Duma (the upper and lower houses of the Russian Parliament), State Social Insurance Fund, Ministry of Health, Ministry of Labour and Social Protection, and other officials. On this occasion, discussions were held regarding issues of quality and control of medical care, the legal, social and economic aspects of the provision of technical tools for the rehabilitation of post-op patients, particularly the issue of the complex approach to speech and hearing rehabilitation for patients after a CI procedure, ensuring access and uniformity of technological standards and quality criteria for rehabilitation across the whole territory of Russia. Valery Ryazansky, the Chairman of the Federation Council's Committee for Social Policy, stressed that it is essential for the state to take an active part in the rehabilitation of children who have received implants. At present in Russia, the state takes care of the cost of providing children with cochlear implants, but after the operation, the parents have to pay for medical rehabilitation themselves. Separately, a whole series of issues was raised regarding the protection of the rights ot people with hearing loss. "There is a very delicate set of social guarantees". Yuri Yanov explains, "A person who has undergone a cochlear implant procedure and successfully been rehabilitated can theoretically be deprived of the social benefits that they depend on and are used to," At the congress, there was о discussion of the legal, social and economic aspects of providing technical rehabilitation tools, "The participation in the congress by representatives of the Federation Council, various ministries and departments is a guarantee that all of the issues raised will be resolved", Yuri Yanov adds.
The value of the Congress in terms of developing CI in Russia
The value of this Congress cannot be overemphasized. First of ail, it provides the opportunity for an exchange of experience, including at an international level, as the Congress was attended by specialists from leading cochlear implant canters from various countries around the world. Secondly, thanks to the large participation of ENT doctors, speech rehabilitation specialists, audiologists and teachers of the deaf from all corners of Russia, it enables knowledge of this experience to spread to the regions. And thirdly, it is a way to strengthen the regulatory and legislative situation with the results obtained, with the help of governmental representatives who took part in the Congress.
"With the current support from various committees of Parliament, the Ministry of Health and regional authorities, the future looks bright for cochlear implants", Yuri Yanov sums up. "Over a short period of time we have been able to catch up, in this area, with the level of the leading Western powers. Now, our task is to ensure that there is no drop in the results we have achieved, and that we keep moving forward,"
The 2nd All-Russian Congress was another step towards the unification of professionals and the development of high-tech treatment for people with hearing loss, having covered the vital issues of rehabilitation for patients with implants.
